Now accepting applications for Community Based Care Coordination Solutions

The North Central Accountable Community of Health (NCACH) is accepting applications from local and regional partners for new or enhanced community-based care coordination solutions that break down silos and improve health equity across our North Central region.

Community-based care coordination is a relational and person‐centered approach that deliberately organizes and facilitates access to comprehensive social and healthcare services in order to facilitate the well-being of individuals and those who care for them.

NCACH is particularly interested in approaches involving:

  • Community-based care coordination/navigation services delivered by a dedicated community-based workforce

  • Efforts led by organizations that are cornerstones of their community to strengthen the network of community-based and social service organizations through expanded partnerships and increased collaboration

  • Efforts to organize and align services and programs at a “one-stop shop” that is convenient for those most in need

This funding opportunity is open to partners, especially community-based organizations and social service agencies, with an existing physical presence in Chelan, Douglas, Grant, and Okanogan Counties. Partners currently receiving funds through our Community-Based Care Coordination project are not eligible for this particular funding opportunity.
