NCACH Governing Board approves funding for local health initiatives, to be managed by the Coalitions for Health Improvement

December 3, 2018 --  NCACH's Governing Board passed a motion allocating $450,000 to be invested in local and regional health initiatives in 2019.Project criteria and funding allocations will be managed by NCACH's three Coalitions for Health Improvement (CHIs). Funding is intended to strengthen community-clinical linkages, address the social determinants of health, reduce barriers to health, and promote wellness. More information about applying for funding will be shared as it becomes available.The Coalitions for Health Improvement advise, inform, and identify solutions to improve the health and wellness of their communities as a part of North Central Accountable Community of Health's Medicaid Transformation Project efforts, serving Chelan, Douglas, Grant, and Okanogan Counties.To learn more about the CHIs, please visit: https://ncach.org/chiFor questions about the approved funding, or the Coalitions, please contact Sahara Suval, sahara.suval@cdhd.wa.gov
